2022 - 2023 Team Staff Application - have volunteered with GPMHA before.
2022 - 2023 New Team Staff Application- if you are new to GPMHA.
2022 - 2023 Signed Police Information Check- take with you to the RCMP station.
(must be done every 3 years - we must have on file)

Please note that due to the nature of our economic region, we do ask that our assistants obtain the same certifications as Head Coaches. This will enable quality instruction if one or more of the team staff are on call or away on shift.
